Home and Community Based Waiver
Available through Medicaid. ADRC does assessment: need help with transferring, walking, bathing, dressing, toileting, and feeding (need 3 of these to qualify). Medicaid doesn’t pay for cleaning, cooking, shopping, transportation to appointments.

Care Transitions Program
Transferring from care facilities back home. Provide input, help to get resources they need and get Institutional Care waiver.
